ПРОБЛЕМИ ФІНАНСОВОГО ЗАБЕЗПЕЧЕННЯ ОХОРОНИ ЗДОРОВ’Я В УКРАЇНІ В КОНТЕКСТІ ОПТИМІЗАЦІЇ СПІВВІДНОШЕННЯ МІЖ СПРАВЕДЛИВІСТЮ ТА ЕФЕКТИВНІСТЮ
Анотація
У статті розглянуто роль оптимізації співвідношення між справедливістю та ефективністю як важливого чинника розбудови механізму фінансового забезпечення охорони здоров’я. Обґрунтовано, що досягнення такої оптимізації сприятиме визначенню пріоритетів державного і приватного фінансування як основи формування адекватної моделі фінансового забезпечення охорони здоров’я. Досліджено загальну концепцію вирішення завдання розподілу державних ресурсів між різними напрямами на прикладі використання кривої виробничих можливостей, яка відображає різні співвідношення обсягів ресурсів, що спрямовуються в сектор охорони здоров’я та в інші соціально-економічні сфери. Наголошено, що обмеженість ресурсів актуалізує проблему раціонального вибору між обсягами коштів, що виділяються на охорону здоров’я та на інші види економічної діяльності, а звідси і на визначення оптимального співвідношення між обсягом безкоштовних медичних послуг, які фінансуються за рахунок державних ресурсів страхових внесків, й обсягом медичних послуг, що купуються за особисті кошти населення в приватному секторі охорони здоров'я. Визначено, що «провали ринку» обумовлюють необхідність присутності держави в сфері охорони здоров'я, а «провали держави» доцільність функціонування приватних медичних закладів. Доведено, що для оптимізації співвідношення між ефективністю та справедливістю у сфері фінансування охорони здоров’я, важливо спрогнозувати та кількісно оцінити результат збільшення або скорочення витрат на охорону здоров'я, що дозволить визначити пріоритети державного фінансування. Наведено підходи до розв’язання проблеми, що стосується визначення такого обсягу медичної допомоги, що надається населенню безкоштовно за рахунок коштів бюджету і обов'язкового медичного страхування, за якого буде дотримуватися принцип справедливості та підтримуватися економічна ефективність як галузі охорони здоров'я, так і державного сектора в цілому. З’ясовано, що ступінь дотримання принципів справедливості та ефективності залежить від комбінування рис бюджетної, страхової і ринкової моделей фінансування охорони здоров'я, які по різному впливають на доступність медичної допомоги, а відтак, і на стан здоров'я населення.

PROBLEMS OF HEALTH CARE FINANCIAL SUPPORT IN UKRAINE IN THE CONTEXT OF THE EQUITY AND EFFICIENCY CORRELATION OPTIMIZATION
Summary
The article is devoted to the analysis of equity and effectiveness correlation optimization as an important factor in the development of the health care financial security mechanism. It is substantiated that the achievement of optimal correlation will help to determine the priorities of public and private funding as the basis for the formation of an adequate model of health care financial support in Ukraine. The general concept of solving the problem of distribution of state resources between different directions is explored, for example, using the production capacity curve, which reflects the different ratios of resources allocated to the health sector and other socio-economic spheres. It is stressed that resource constraints highlight the problem of rational choice between the amount of funds allocated for health care and other types of economic activity, and hence the determination of the optimal ratio between the volume of free medical services financed by state resources of insurance premiums, and the volume of medical services purchased for personal expenses of the population in the private sector of health care. Defined that «market falls» substantiates the necessity of state regulation in heath care, and «state falls» is the reason for private medical institutions to be present on the market. It is proven that in order to optimize the relationship between efficiency and equity in health financing, it is important to predict and quantify the result of an increase or decrease in health care spending, which will determine the priorities of public funding. Shown the lines to solve the problem of the amount of free state-guaranteed healthcare services that will guarantee both aspects - theprinciple of equity still works and the healthcare system is efficient in economic terms. It is established that the degree of observance of the principles of justice and efficiency depends on the combination of features of the budget, insurance and market models of health financing, which have different effects on the availability of medical care, and hence on the health of the population.
